Giant hemangioma with thrombocytopenia. Radioisotopic demonstration of platelet sequestration.

چکیده
T HE ASSOCIATION of thrombocytopenia with giant hemangioma has been documented in approximately 40 cases since the first report of Kasabach and Merritt in 1940. The mechanism of thrombocytopenia is thought to he related to the trapping of platelets by the abnormal endothelium of the tumor, and the accumulation of material presumably derived from the platelets has been demonstrated by histologic technics.3 The actual sequestration of platelets by the tumor should be easy to verify by transfusing platelets labeled with a radioactive isotope, but such studies have produced conflicting results. Petit2 has claimed that no platelet sequestration takes place, and Blix and Aas7 found no increased radioactivity over the tumor following the infusion of tagged platelets. Kontras et al. performed careful studies with baseline calculation of the tumor vascular space followed by infusion of Cr-51 tagged platelets. These authors concluded that the increased radioactivity over the hemangioma should be atrihuted not only to the large blood volume present in the hemangioma hut to an actual trapping of the platelets.6 The present report describes a case of thrombocytopenia associated with giant hemangioma in which similar studies were performed. While our tracer studies are somewhat different from those of Kontras, our isotopic findings, together with the radiotherapeutic results, tend to confirm the occurrence of platelet trapping as reported by these authors.

Increased platelet destruction in infancy and childhood.
The number of platelets in the peripheral blood of healthy children, full-term, and premature infants falls into the same range as that of adults. Thrombocytopenia, defined as a platelet count of less than 150,000/mm(<150 × 10/L), may be due to decreased production, splenic sequestration, or increased platelet destruction. The following is a review of disorders involving the last mechanism, all...
متن کاملBaseline thrombocytopenia complicated by recurrent episodes of transient severe thrombocytopenia following infections in an adult woman with a non involuting congenital hemangioma – a case report
BACKGROUND Congenital hemangiomas are benign abnormal proliferation of blood vessels. Noninvoluting congenital hemangiomas are a rare variant which persist, and may become bigger. Hemangiomas are known to be associated with thrombocytopenia, microangiopathic hemolytic anemia and Kasabach-Merritt phenomenon. Kasabach-Merritt phenomenon is characterized by consumptive coagulopathy with microangio...

Hemangiomas of infancy are usually congenital lesions. Occasionally they are associated with microangiopatbic bemolytic anaemia, thrombocytopenia, and a consumptive coagulopathy. This is most often associated with cavernous hemangiomas. Thrombocytopenia associated with giant cavernous hemangiomas was first noted by Kasabach and Merritt in 1940. As the primary mechanism of platelet destruction i...

Giant chorioangioma is a placental tumor associated with gestational complications such as preeclampsia, polyhydramnios and hemorrhage. In addition, this tumor might lead to the incidence of non-immune fetal hydrops, heart failure, anemia, thrombocytopenia, weight loss and death among neonates. In this case report, the clinical image of a term newborn (weighing 2800 g) with one-minute Apgar sco...
